One of the key issues in the supply chain is the coordination between its components with the aim of customer satisfaction. To achieve this goal, supply chain risk factors must be identified so that they can be managed. The purpose of this research is to identify and classify the risk factors of the supply chain of the pharmaceutical industry. In this research, meta-synthesis methods have been used to identify and analyze the importance-performance for the ranking of risk factors. Also, the approach of this research is a combination of qualitative and quantitative methods. In the first step, with a systematic review of related studies in the last 23 years of the synthesis approach, the risk factors of the supply chain of the pharmaceutical industry were identified. For this purpose, first 100 articles were identified and after several stages of review and filtering, 26 articles were used as the basis for the research. In the next step, the identified risk factors were ranked with the importance-performance analysis approach. According to the results, the risk-creating factors of the supply chain of the pharmaceutical industry, in order of importance and priority, are: 1- Political, legal and environmental factors (sanctions, strikes, natural, war and terrorism, weather, environment, etc.) ...) 2- Currency supply and foreign financial payments 3- Fluctuations in imports and lack of timely supply of raw materials 4- Complexity and incompatibility of information systems (disruption of customer service system) 5- Low quality of raw materials and products 6- Unavailability of medicine 7- Transportation and insurances 8- Bankruptcy of key supplier 9- Improper management, planning and control of production and inventory 10- Increase in the price of raw materials. From the ten risk factors obtained, political, legal and environmental factors (sanction, strike, natural, war and terrorism, weather, environment, etc.); Supply of foreign currency and foreign financial payments, fluctuations in imports and lack of timely supply of raw materials have a greater effect on the supply chain. Therefore, the stakeholders of Iran's pharmaceutical supply chain should focus on these more important risk factors and plan for their management in order to improve the performance of the supply chain.
