Risk in the supply chain is a potential event that disrupts the natural flow of materials and information, leading to interruptions within the chain. A wide range of risks present in supply chains may negatively affect their performance. Due to the close interconnection among supply chain members, the occurrence of a disruption or in other words, risk in any part of the chain can influence the entire system and hinder its performance. Therefore, organizations need to adopt appropriate strategies for managing and controlling supply chain risks. This study aims to identify and prioritize the risks of the steel industry supply chain under conditions of uncertainty by employing Fuzzy Hierarchical Analysis and Type -2 Fuzzy TOPSIS. Accordingly, the research method is a mixed exploratory approach (qualitative–quantitative). The results indicate that the top three risks in the steel industry supply chain are, respectively, the risk of raw material suppliers’ performance, production planning risk, and the risk of information system misalignment. It is recommended that a comprehensive and executable plan be designed and implemented to effectively control and monitor these risks.
